Ageing with Choices offers End of Life Doula Services to guide you through the complexities of ageing and end of life decisions.

We offer compassionate, non-medical, non-clinical support to you, your family, and your friends in a safe and understanding environment.

We provide comprehensive education on key topics such as ageing, end of life planning, care options, the stages of dying, what to expect during this time, and funeral planning.

Our advocacy services are designed to empower you, ensuring your voice is heard and your wishes are respected throughout the process.

We help coordinate services you need to live your final years in alignment with your desires.

Ageing with choices facilitates open communication between you, your family, your friends, and healthcare providers, ensuring everyone is on the same page and working together in support of your end of life journey.

We also assist navigating end of life services, including My Aged Care, home care, palliative care and voluntary assisted dying.

Ageing with Choices offers End of Life Doula services to individuals, as well as to groups through informative sessions tailored to community organisations and workplaces.

Accessing Home Care Services

 

Ageing with Choices can help you access Home Care Services through the My Aged Care system.

The Commonwealth Home Support Programme provides entry-level care, supporting your ability to live independently at your home.

For more complex care needs, Home Care Packages offer additional support while you remain in your home.

These services are accessed through an assessment via the Single Assessment Pathway.

Based on your assessment, services may include domestic assistance, personal care, home modifications, meals and food services, allied health and therapy services, nursing care, home maintenance, equipment and assistive technology, respite, transport, and social support.

Ageing with Choices can guide you through the entire process - from registering with My Aged Care to implementation of these services in your home.

Some people require professional care that can only be provided in a Residential Care Facility. Making this decision can be an incredibly emotional process, both for the person involved and their family.

Ageing with Choices is here to offer support and guidance as you navigate this decision.

We can assist with finding a suitable facility, negotiating fees, completing necessary paperwork, and if needed be there for the admission process.

Wellness Conversations

Wellness Conversations offer a chance to sit down and have a meaningful chat. This service can be provided in a one-on-one setting or within a group environment.

If you’re feeling lonely, anxious, sad, or worried, sometimes just talking to someone can help ease those emotions.

Maybe you’re caught in a behaviour you’d like to change, or there’s something new you’d love to start but are struggling with motivation.

Would you like to practice more mindfulness or gratitude? Perhaps you’re looking to lose weight, increase physical activity, or simply bring more movement into your routine?

Or maybe you’re just missing the connection that comes from good, healthy conversation?

End of Life Conversations

 

End of Life Conversations are a service offered by Ageing with Choices.

End of life is a natural part of the life cycle, something we will all face at some point.

At Ageing with Choices, we offer this service because many people find it difficult to talk about what the end of their life may look like. Others may feel ready to discuss it, but their loved ones aren’t emotionally prepared or open to such conversations.

Are you ready to discuss your end of life wishes or address any unfinished business that’s been weighing on your mind? Maybe you just need to voice thoughts and feelings you’ve been holding onto, seeking a sense of peace and contentment in your life.

Ageing with Choices provides a completely confidential space for you to share whatever is on your mind, without judgement.

Please note, Ageing with Choices supports each person’s right to make decisions about how they live their final chapter. However, we do not provide assistance on how to end one’s life outside the scope of exploring voluntary assisted dying.
